Our PRP for Adults offers various services to individuals diagnosed with mental health illnesses to keep them functional in the community while decreasing homelessness and promoting stability. The main scope of our adult services is to work with clients on various goals in the areas outlined below:
We assist our clients and their families with any housing needs. We work with our clients to develop a housing plan and home search. We work with our clients throughout the program to achieve their financial security goals and obtain independent housing in their chosen community.
We meet with our clients to identify vocational interests and goals. Then, we guide our clients through employment training, assisting with mock interviews and resume-building, monitoring client progress toward vocational goals, and processing workplace issues. Additionally, we help our clients pursue independent employment by assisting with job applications and follow-up.
We help our clients identify their transportation needs and barriers to independent travel.
We help our clients identify educational interests and goals. In addition, we guide our clients throughout academic training and teach them to seek additional educational resources independently.
We help our clients identify ways they can address financial needs, such as entitlements, financial assistance, and budgeting. For example, we work with our clients to access entitlements and financial assistance and to follow up with applications and recertification dates. In addition, our trained field staff helps individuals achieve financial security by developing a budgeting plan.
We work with our clients to ensure they actively participate in therapy and maintain contact with counseling services as needed.
We help our clients identify substance abuse, mental health, and physical health needs. Additionally, we work with our clients to connect with treatment providers to address these needs. Finally, we collaborate with providers throughout the program to ensure we address clients' needs.
